I am not sure if this applies to you or not, but I have had similar issue (though I caught it before the attack sent) and I found the root cause:

When the notification popped up in unit-select screen, I was accidentally mashing the "select all" button (usually select-all Espionage), because of the way the UI is on my screen, instead of hitting "Send".

There may be something that can be done to improve this area of the game, but (for me at least), it's not anctual bug
