I've noticed that having the "Warmaster" mastery (probably "Giant slayer" as well) can prevent "block revive" skills from working.
This is because the champion is being killed by the warmaster damage and not by the skill. 
I've noticed this with "Fenax" but I assume all other champions with block revive skills have the same problem.
