I have had a problem getting 3 stars on nightmare despite nether of my two champions having died, at fist I thought nightmare didn't like me resurrecting champions (though I did get 2 stars still) but I have done several with zero deaths and still can't get the third star I will update this with a list of the ones I can't 3* as soon as I can
