Hello!
To get three stars you need to beat a stage with two champions without losing a single one in the process.
This information is available in the Info tab (i) of the Campaign window.
We're now working on the UI improvements for the Campaign window and will add the information on the requirements to the main screen.

Hi!
I've mentioned in the previous reply that a player can find the information on the requirements in the Campaign window.
In addition to using two champions you also need to beat a stage in less than 10 minutes.
Most likely, the battle was longer then 10 minutes and this is why you didn't receive third star.
