No you do not, you are not guaranteed to get runes when you attack ghosts, in fact its about 50% runes and  being polite,50% the rest.

Runic knives(they drop from ghosts,or cost gold), are needed to remove runes from equipment, you have to have legendary class equipment to insert runes in the space reserved for them.
