So after logging into both Firefox and Chrome trying to troubleshoot this issue, I still showed the Class IV rare offensive talisman with one upgrade missing in my general's inventory.

However, I did notice that each time a talisman disappeared, another talisman I previously owned would appear in its place.  So I had the idea to move the Titan's Mask to an empty slot from where I last saw the Class IV rare offensive talisman with one upgrade (I now know to be called the Patera of Pygmalion).  In doing so, after a browser refresh, I can now again see my Patera of Pygmalion with one upgrade.

I moved the Titan's Mask from the red circled slot, down to the (at the time) empty slot circled in green, leaving the slot circled in red empty, and after doing so, my Patera of Pygmalion reappeared after refreshing my browser.

Initially, it was the Urn of Theseus (with one upgrade) that had disappeared last month.  Followed by it's reappearance a few days ago and the disappearance of the Patera of Pygmalion (with one upgrade).  The Patera of Pygmalion (with one upgrade) reappeared last night after a few days of not showing up in my general's inventory, only to have disappeared again today when I got home from work.  After moving the Titan's Mask and refreshing my browser, all talismans I can think of are showing again.

Hopefully my issue is resolved, but I will leave this post up as I'm not sure if it is a browser or game mechanics issue.

My apologies, I will follow the instructions you listed.  As I think it may be an ongoing bug, because at one point, I had completely filled my general's inventory as well as my overflow inventory, and my Urn of Theseus (with one upgrade) was still missing.

Platform: Google Chrome Browser

The feature that causes the issue:  Swapping out general armor while doing positions

The exact steps you performed before the bug arose:  I am uncertain.  I was doing many positions whilst swapping out general gear depending upon the units I scouted at each position.

Any special conditions that caused the bug to appear:  As stated above, after one talisman reappeared and another disappeared, I did a little experimenting and was able to make a missing talisman reappear after moving around some talismans to empty slots and refreshing my browser on both Chrome and Firefox.

What should have happened: No disappearance of general items.

What actually happened:  First one talisman disappeared, a month or so later it reappeared, than a different talisman disappeared from general's inventory.  Then the current missing talisman reappeared, with another disappearing in its place.  After some experimenting, I was fortunate and able to move a talisman and making the current missing talisman reappear.
