After testing, I have to change my previous answer, and have now know what happens:
Your Conquerer died.
The blue bar under his pic is full, meaning he has taken full damage to his full HP, which means he didn't survive, hence the 1 star
Actually full blue bar does not mean he died.The bar is always full for the champ who took the more damage, heal the most or did the most damages.
But your screen do not show that your whole team survived. If you win the stage, it is never written "dead"
Just to be sure it is clear for you (en claerly written in the campaign):
1star = victory
2stars = victory no loss
3stars = victory, no loss and only 1 or 2 champions

Although I think you're right about the blue bar, I still stand by my guess that Conqueror is dead.
And also to be clear, the picture in the link is Faction Wars, NOT campaigns where the condition are as follows:
1* = clear stage
2* = clear stage using 3-4 champs, no deaths
3* = clear stage using 5 champs, no deaths

Actually no, I have test it again
Red bar = damage dealt
Blue bar = damage received
Green bar = healing cast
There's actually no indication whatsoever who died

Well of course, that's why 2 of them died. My point is, you can't tell which one is dead.
Red is how much each champ hit
Blue is how much each champ get hit
Green is how much each champ cast heal onto themselves or to their allies
