When you sacrifice a champion, does the gear it was wearing disappear with it or does it just unequip?

Hey, Xyphire. The Artifacts disappears if you sacrifice a Champion that wears them. We are currently planning to add a warning notification that will pop-up when a player attempts to sacrifice a Champion with any Artifacts on them.

I assume that the skills and everything that you upgrade on the creature/champion that is sacrificed is lost, is that correct?

Yes, you will lost all your Books wich you used for upgrade that champion's skills, Items and Masteries.
You can lock champions for don't sacrifice them for errore.

One year later and still no warning.
Here’s a better idea. Why don’t you just change a very obviously bad game policy.
