Questions 1) Do you have artifact (or in this case accessory because there are accessories in this pass) space?
2) If those are full, do you have inbox space?
3) Are you positive you're checking for the right artifact types?
4) You've earned enough points to collect the forge pass rewards, correct?
If the answer to all those are yes, you should open a support ticket.
