As above, I have "claimed" him two times now, only to be not be to find him.... the second time was less than a hour ago, the silver key icon had a "2" below it, and the Hellmutt had a "3" below it (in the place where the blessing icon shows up).

Those are fragments. You need to clear 34 Secret Rooms in Normal Doom Tower secret rooms to be able to summon him from the necessary 100 fragments, in the summoning portal.
