I recently put Nethril in destroy gear for the scarab boss and after I removed the gear and swapped in new gear, nethril loses 5,000 HP every time he attacks in any scenario regardless of gear. It hasnt done this with anyone else except nethril, but now he is unplayable. It is a 5k hp loss per hit, so on his multihitter it is 5k x number of hits. This is horrible
I don't understand what you are saying? Are you saying when you attack, you are taking damage when you attack? If so, specify which wave you are on. Some champions have a passive which gives damage based on your attack. I think this is what you might be seeing. It has nothing to do with your gear.

I believe it is the passive of Elder Skarg. He is in some of the later
