Mainly happens because even with block buffs, he still steals tm from his target. That means he is getting more turns than the other heads, which makes him more likely to be the one who eats.
The two most likely eaters are mischief and suffering - the latter because of his double turn.
Also remember that decay can't eat if provoked, and that blight will prioritize toxic cloud over eating. I think wrath will also favor provoke, and a few other similar situations.
All adds up to those two being the most likely eaters.

I think decay still eats when provoked? But I am still always guessing when my champs will get eaten ... the mechanics of skipping or eating while counter is at 0 are a mystery to me.
Definitely does not. The skipping when counter at zero happens when the the head which is up next to eat has a higher-priority move to use.
I know for sure that Decay will skip when provoked. The others I'm not totally certain about - would need to catalog some test cases.
